24-Hour Emergency Assistance

24-hour emergency assistance — listed in Minnesota Statutes 245D.03 as a basic support service and defined under the CADI and DD waiver plans as 24-hour emergency assistance, personal emergency response — gives a person who lives in their own home a reliable way to reach trained help at any hour. Support is arranged in the person's coordinated support plan and is limited to counseling, consultation, and response to keep the person safe; it does not include training or habilitation.

What's included

  • On-call staff reachable by phone 24 hours a day, every day of the year
  • Telephone counseling and consultation to talk a person through an urgent situation
  • In-person response at the home when phone support is not enough
  • Back-up and emergency plan written with the person and their case manager
