Individual Community Living Support (ICLS)

Individual community living support is a basic support service under Minnesota Statutes 245D.03 and is established in Minnesota Statutes 256S.13. ICLS bundles the assistance and cueing an Elderly Waiver participant needs across the day so they can keep living in a home they control. By law, ICLS must be delivered in a single-family home or apartment the participant or their family owns or rents under a lease, the provider may not be the participant's landlord or hold any interest in their housing, and the case manager writes an individual community living support plan with the participant using a state tool.

  • Reminders and coordination for appointments, medications, and daily schedule
  • Delivered in the participant's own owned or leased home or apartment
  • Provider cannot be the landlord and has no interest in the participant's housing
  • Follows an ICLS plan the case manager writes together with the participant
