Individualized Home Supports with Family Training

Individualized home supports with family training is an in-home support service under Minnesota Statutes 245D.03, subdivision 1(c)(2), defined under the BI, CAC, CADI, and DD waiver plans. The training is delivered to the person's family members and unpaid caregivers so they can support the person more effectively at home, and it is tied to the outcomes in the coordinated support plan.
