Individualized Home Supports with Training

Individualized home supports with training is an in-home support service under Minnesota Statutes 245D.03, subdivision 1(c)(2), defined under the BI, CAC, CADI, and DD waiver plans. Unlike basic individualized home supports, this service is specifically directed toward training and habilitation: staff teach and coach the skills written as outcomes in the person's coordinated support plan, and progress is documented and reviewed.
